Departments are to keep written records of operator training and evaluations.  Training provided by WSU can be documented on this form. Companies conducting training are to provide appropriate training records.

The documented training and evaluation records must include, at a minimum, the following:

• Name of the operator
• Date of the training or training
• Name of the person giving the training or evaluation

Training records are to be maintained in accordance with the WSU retention schedule (BPPM 60.71).
